Close Account Form
This form is for your current financial institution. In order to finish transferring your account to Horizon all you need to do is complete this form and mail it to your current financial institution.
Direct Deposit Form
To change the direct deposit of your paycheck to Horizon Credit Union, simply complete this form and bring it to your employer's payroll office.
Automatic Withdrawal Change Form
Complete this form and submit it to all creditors that are automatically withdrawing funds from your existing checking account. This form may be copied and used as many times as needed.
